Health-Supported Housing

Rooted in Grace Foundation provides housing and support services in small-scale, trauma-informed residential communities where pregnant and parenting women and their children can heal, stabilize, and build long-term well-being.

In addition to safe, stable housing, all residents receive the following holistic support:

We eliminate barriers to care by providing non-medical transportation via passenger minivans, direct AHCCCS and SNAP enrollment assistance, and vital safety items like car seats, diapers, and SNOO Smart Sleeper bassinets.

We deliver trauma-informed behavioral healthcare that integrates Medication-Assisted Treatment (MAT) with evidence-based interventions such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and the SMART Recovery program. Residents are guided by certified Peer Support Specialists who leverage their lived experience to coach mothers toward long-term stabilization and relapse prevention.

We utilize the evidence-based Triple P (Positive Parenting Program) to help mothers build attachment, develop positive discipline strategies, and create structure in recovery households.

Our innovative workforce pipelines include Thrift to Thrive (e-commerce micro-business training) and Homegrown AI (digital entrepreneurship), equipping mothers with 21st-century skills for home-based income generation.

We ensure children’s developmental needs are met through on-site early childhood education, developmental screenings, and attachment-based parent-child activities.

Through our partnership with the Maricopa County Office of Public Defense Services, we provide pre-dependency legal advocacy to prevent unnecessary child removals, reduce trauma, and support family reunification.

We provide specialized 6 to 24-month transitional housing and stabilization services for survivors of sex and labor trafficking, ensuring long-term safety from re-exploitation.

Our housing communities are intentionally designed for healing, recovery, and opportunity. Preston Place and The Lily House are safe places where mothers and young children can rebuild their lives.

Both of our active residential facilities are in walkable, mixed-use neighborhoods with access to public transportation, parks, and healthy food. To ensure the protection of the mothers and children in our care, all residential housing addresses are confidential.
