Heal & Home AmeriCorps Program
Rooted in Grace Foundation, in partnership with the Governor’s Office of Youth, Faith, and Family, operates the Heal & Home AmeriCorps program. Our AmeriCorps members are not volunteers—they are trained, compensated leaders integrated into our multidisciplinary care delivery system.
As a Heal & Home AmeriCorps member, you will serve as a vital link in addressing disparities in healthcare access, mental health services, and child well-being for low-income families with young children (aged 0-5 years) in metro Phoenix. Serving 32-34 hours per week, you will deliver evidence-based public health education, teach emotional resilience, and navigate complex social services to foster a trauma-informed environment where families can thrive.

Qualifications*
- Female applicants preferred
- A high school diploma is required
- College degree or equivalent life experience preferred
- Valid driver’s license
- Candidates are ideally 21 years of age and older
- Ability to lift 30 lbs.

Responsibilities
As a Rooted in Grace Foundation AmeriCorps member, you’ll have the opportunity to undertake a range of responsibilities to foster a secure, nurturing environment for our patients and residents.
- Health-Supported Parenting Education: Conduct 1-on-1 in-home sessions utilizing the evidence-based Triple P Parenting framework to teach positive discipline, child development, and stress management techniques.
- Healthcare Referral & Navigation: Conduct social health screenings (using tools like PRAPARE) to identify mental health or substance use concerns, assist families with insurance enrollment, and connect them directly to medical providers.
- Screening & Resource Linkage: Identify critical basic needs and provide warm referrals for housing, food security, legal assistance, and transportation.
- Collaborative Team Projects: Participate in one of three specialized teams to strengthen our community footprint: Amplify (Communications & Recruitment), Service (Events & Celebrations), or Connections (Community Outreach & Partnerships).
