The Challenge

Across Arizona, pregnant and parenting women face compounding risks that traditional healthcare systems are not designed to resolve on their own. These risks include:

  • Rising pregnancy-related deaths
  • Untreated mental health and substance use conditions
  • Housing instability during pregnancy and early parenting
  • Gaps in prenatal, behavioral health, and recovery care

Without early, coordinated intervention, these challenges escalate placing both mothers and babies at unnecessary risk and increasing long-term system costs.

Our Solution: Health + Home

At Rooted in Grace Foundation, our coordinated care model combines safe housing with integrated, trauma-informed services. By addressing stability first, mothers can engage meaningfully in care, strengthen their parenting capacity, and build sustainable independence.

Through our residential communities and integrated services, we provide:

Safe, stable housing facilities

Integrated behavioral health and recovery support

Mental health and substance use treatment

Peer support and Community Health Worker services

Parenting education and family strengthening

Workforce readiness and economic stability support

Early childhood development and care coordination

This model allows families to build stability and resilience, before emergencies occur, and supports long-term well-being for both mothers and children.

As someone who cares deeply about the health, stability, and future of families in our local Phoenix community, you know that real healing begins not in an exam room, but in safe homes, strong relationships, and early, compassionate support—long before a crisis occurs.

Across Arizona, pregnant and parenting women face intersecting challenges—including mental health needs, substance use, housing instability, and gaps in care—that traditional healthcare alone cannot resolve. Rooted in Grace exists to meet families at this critical intersection.

Through our Health + Home model, we combine safe, stable housing with integrated behavioral health, recovery support, family strengthening, workforce readiness, and early childhood services. This whole-person approach addresses the social conditions that shape more than 80% of health outcomes and helps families stabilize before crises occur.
